Ontario Soccer Forms

A Player registered on an Under-14 or older aged team may play for a team to which they are not registered only under one (1) of the following conditions:

a) They play with a “Temporary Eligibility Permit” (TEP): (Currently registered with a Club affiliated to Ontario Soccer or Canada Soccer.)

i. plays up in a League game for a higher-level Club Team of a Club other than one (1) with which they are registered,

ii. plays as a guest Player in a tournament (subject to tournament rules) or exhibition game (subject to the agreement of both teams) with a Club other than one (1) with which they are registered

Forms will now be completed online following this workflow.  I think clubs can eliminate the player/parent step given my first look at the form and it can be completed by the coach/club requesting.

  1. Player/Parent Submission – The form is initially completed and submitted by the player or their parent/guardian.
  2. Club Approval – The submission is routed to the appropriate Club for review and approval.
  3. District Approval – Once approved by the Club, the form is forwarded to the District for final approval.
  4. Distribution - Copies of the TEP are sent to the player, both Clubs, and the District(s) - the attached PDF must be printed and brought to the game/tournament.
